Meet the Team
Dr. Lee Stevens
CLINICAL SUPERVISOR
Throughout his career in the medical profession, he has served his country for over 25 years. He served in both Iraqi wars in the field of emergency medicine. Dr Stevens retired from the army reserves in 2009 with the rank of colonel. Dr stevens married on July 18, 2009, which happens to be his birthday. He has four children and 10 grandchildren. He moved from Shreveport, Louisiana, where Dr. Stevens served as professor at LSU. In July of 2019 he decided to take a much-needed position. Once again, he gave his services to our country by helping at the naval medical hospital in the department of pain management. He began helping our troops with chronic mental health conditions. Dr. Stevens remains active in the field of psychiatric medicine.
Jake Funderburk

Jake is a Licensed Professional Counselor and Executive Director of North Louisiana Whole Health Treatment Center. He works to build a treatment environment that is ethical, accountable, and grounded in evidence-based care for individuals with substance use and co-occurring mental health needs. With experience in both clinical and leadership roles, he focuses on strengthening programs, supporting staff, and helping clients navigate treatment with clarity, dignity, and structure. He has a particular interest in substance use treatment, co-occurring disorders, and the practical work of building programs that balance clinical quality with accountability. His approach is shaped by evidence-based practice, time-tested clinical principles, and a genuine belief that people can change and should never be reduced to a diagnosis. Outside of work, he enjoys reading widely, backpacking far enough into the mountains to lose cell service, and keeping up with the local live music scene.
Leslee Blake

Leslee B. Blake is dedicated to helping individuals who struggle with addiction and approaches her work with the belief that every person has the capacity to change. She is committed to meeting clients with hope, encouragement, and steady support, often reflecting the philosophy, “I can believe in you until you can believe in yourself.” Leslee was credentialed in California as a CADC II Clinical Counselor in 2009 and brings 18 years of experience in residential and intensive outpatient treatment settings. After moving to the South in 2020, she continued her work in the field and is currently credentialed in Louisiana as an RAC and ICADC. Her clinical approach is informed by Motivational Interviewing, Cognitive Behavioral Therapy, and Dialectical Behavior Therapy, and she is passionate about helping clients build the insight and confidence needed for lasting recovery. Outside of her professional work, Leslee values family, faith, and advocacy for animals.
Tiffani Williams

Assessment and Admissions Clinician
Tiffani is a Licensed Professional Counselor with a Master of Science degree and experience providing both individual and group counseling across inpatient and outpatient settings. She works with children, adolescents, and adults and has treated a wide range of concerns, including anxiety, depression, bipolar disorder, life challenges, life transitions, and co-occurring mental health and substance use disorders. Her clinical approach is grounded in person-centered care, Cognitive Behavioral Therapy, and Solution-Focused Brief Therapy, allowing her to meet clients with empathy while helping them build practical skills for change. Tiffani strives to create a space where clients feel safe, heard, and genuinely understood, while empowering them to build confidence, challenge limiting beliefs, and recognize that their past does not define their future. She believes growth, healing, and purpose remain within reach for every client. Outside of her clinical work, she enjoys volunteering at nursing homes and playing bingo with residents, and a childhood passion for the piano.
Jeff Darby

Jeff is a Licensed Professional Counselor, Licensed Marriage and Family Therapist, and Licensed Vocational Rehabilitation Counselor who holds a Master of Arts degree and brings a broad, thoughtful perspective to his clinical work. His areas of focus include working with individuals living with mental and physical disabilities, as well as marriage counseling. Drawing from Cognitive Behavioral Therapy and Solution-Focused approaches, Jeff works from the belief that behaviors are learned and can therefore be unlearned, and that the choices people make carry both positive and negative consequences. He is careful to resist placing limiting labels on people, believing instead in the strength, dignity, and potential of everyone. Jeff strives to treat others as he would want to be treated, recognizing that everyone faces challenges and that helping others is both a responsibility and a privilege. His goal is to empower clients to see themselves as capable of facing difficult life circumstances, to reflect honestly on their own truth, and to leave each session with greater motivation to pursue meaningful and achievable goals. Outside of his professional work, Jeff enjoys spending time near the water, writing short narratives, reading the Bible, teaching Bible study, listening to music, and sharing quality time with family and friends.
Madilyn Lemoine

Madilyn Lemoine is a Provisionally Licensed Professional Counselor who serves as an Intensive Outpatient Program therapist specializing in co-occurring mental health and substance use disorders. Her clinical focus includes behavioral change, emotional regulation, relapse prevention, and identity development in recovery. She approaches treatment from a trauma-informed and client-centered perspective, emphasizing empathy, accountability, and individualized care. Drawing from evidence-based modalities such as Motivational Interviewing and Cognitive Behavioral Therapy, Madilyn helps clients challenge unhelpful thought patterns, build self-awareness, and develop sustainable coping strategies. She believes recovery is not only about abstinence, but also about building emotional resilience, strengthening insight, and creating a meaningful, value-driven life. Madilyn is passionate about helping clients recognize their progress and rebuild their lives in ways that feel authentic and fulfilling, and she strives to create a therapeutic environment where clients feel understood, supported, and appropriately challenged. Outside of her clinical work, she enjoys spending time outdoors with her family, friends, and her two dogs.
Brittany Broussard

Brittany is a substance use therapist with nearly four years of experience in both inpatient and outpatient settings, providing group and individual therapy for individuals navigating recovery and co-occurring disorders. Her work is grounded in the belief that meaningful change is possible when people are given the right support, honest guidance, and room to grow. She strives to create a therapeutic space where accountability and compassion coexist, helping clients confront unhealthy patterns, build healthier coping strategies, and develop a stronger sense of identity and purpose. Using evidence-based approaches such as Cognitive Behavioral Therapy and Dialectical Behavior Therapy, Brittany helps individuals increase self-awareness, regulate emotions, and move toward lasting change with greater clarity and intention. Outside of work, she enjoys reading and writing poetry, which reflects her belief that healing often begins with honest expression and in the courage to face what we feel.

Nika is an IOP Counselor with a Master of Science degree and Licensed Addiction Counselor credential who is dedicated to supporting individuals in their mental health and recovery journeys. Her work is guided by person-centered therapy, Cognitive Behavioral Therapy, and Dialectical Behavior Therapy, allowing her to help clients build insight, develop healthier coping strategies, and move toward lasting change. Nika is especially passionate about uplifting clients and creating a space where they can find peace, confidence, and reassurance during difficult times. She believes deeply in the importance of helping and supporting others, recognizing that everyone needs someone in their corner. Outside of her professional role, Nika absolutely loves being a mom, a role that reflects the same care, commitment, and compassion she brings to her work with clients.
Beth Kirk

Beth is a Licensed Clinical Social Worker who brings warmth, insight, and a relationship-centered approach to her clinical work. She is well versed in Trauma-Informed Care and Systems Theory, and her practice is shaped by person-centered, strength-based, and Acceptance and Commitment Therapy approaches. Beth believes that meaningful relationships are at the heart of lasting change and works to create a space where clients feel seen, supported, and empowered as they move through recovery. She strives to help individuals understand themselves more fully and compassionately while building confidence in their own ability to navigate sobriety and life’s challenges. In addition to her clinical work, Beth is also a school social worker and a devoted wife and mother of two. Outside of work, she enjoys reading as many books as she can, spending time with her family, and caring for her rescue dog, Charlie.
Shantelle Butler

Shantelle brings more than 20 years of military and administrative leadership experience to her work, with a background that reflects exceptional organizational skill, professionalism, and operational discipline. Her experience includes executive administrative support, human resources functions, staff supervision, compliance oversight, training, and process improvement across fast-paced and high-responsibility settings. Throughout her career, she has supported senior leadership, managed confidential records, coordinated complex operations, and helped teams’ function with greater structure and efficiency. Shantelle’s professional strengths include communication, discretion, problem solving, and a steady, detail-oriented approach to supporting both staff and organizational systems. Her background makes her especially well-suited for roles that require strong leadership support, accountability, and thoughtful coordination in demanding environments.
Jason Farles
Marketing and Community Outreach Liaison
Jason is the Marketing and Community Outreach Liaison for Whole Health and Crossroads Recovery. His life experience has given him a grounded understanding of struggle, resilience, and the power of personal change, which continues to shape the empathy and perspective he brings to his work. After overcoming significant challenges earlier in life, Jason experienced a turning point that gave new direction to his path and purpose. He credits faith, accountability, and second chances with changing the course of his life.
Today, Jason uses both professional dedication and lived understanding to help individuals struggling with addiction and alcoholism connect with treatment, sobriety, and recovery support. He is driven by a deep belief that change is possible, even when someone feels they have run out of chances. He finds real meaning in believing in people before they believe in themselves and in helping them take the next step toward recovery. In addition to his outreach work, Jason has been instrumental in several community initiatives, including helping secure food donations for local shelters, building partnerships with community colleges, and strengthening relationships with community policing efforts.
